Login ID:
Your Login ID is the PID found in your Order Form (under "Patient Details").

Password:
Your password is your Last Name (in CAPITAL letters).
If you have name suffixes (JR, II), it will be included in the Last Name.
For example, FERDINAND JOSE JR = JOSEJR

Login ID: This is the unique ID given to you by Health Online.
Password: This is the Password given to you by Health Online.
